Long Term Condition Checks
A long term condition check is an annual (or more frequent, if needed) health check for patients with any long term condition.
These checks are done to make sure that your condition is managed effectively and that you're generally well.
It's important that you come in for your check-up when you're invited to help you stay healthy.

Who is this for?
Anyone with one or more long term condition, such as: diabetes, high blood pressure, heart disease, asthma, COPD, rheumatoid arthritis, dementia.
What to expect
The nature of your appointment depends on your condition and the checks required. These checks could be done by anyone in our clinical team but are normally lead by our practice nursing team. We can tell you what your appointment will involve when you book.
How can I book?
You’ll receive a text or call from us when your check is due to arrange an appointment. Alternatively, you can proactively book your appointment by calling the practice, or via Patient Access or the NHS app.
Covered by NHS?
Yes, a long term condition check is a free health check covered by the NHS.
